(1)	Power of attorney : Write only '3.Mandator'	We send (1),(2),(3) to you.
(2)	Moving and Major Items Declaration : Write the blank except 'Transport Description'
(3)	Descriptions of Declared Items (If you have items, please write all the things)
(4)	Copy of passport
(5)	Certificate of Entry & Exit
(6)	Family Relation Certificate	In case of being family member

 

 

	Citizenship
(1)	Power of attorney : Write only '3.Mandator'	You have to sign on the documents the same as on the passport.
(2)	Moving and Major Items Declaration : Write the blank except 'Transport Description'
(3)	Descriptions of Declared Items (If you have items, please write all the things)
(4)	Copy of passport	You have to bring copies of all the family members.
(5)	Certificate of Entry & Exit
(6)	Copy of Certificate of address (both sides of Certification)
(7)	Family Relation Certificate	In case of being family member
